Physicals
All scouts and adult leaders at Hale must have a completed Boy Scouts of America Annual Health and Medical Record form on file at the Health Lodge. The Boy Scouts of America has established minimum standards for providing medical information prior to participating in various activities. Those standards are offered in one 3 part medical form. All 3 parts need to be filled out. Part B is to be completed and signed by a certified and licensed health-care provider. Physical forms will be returned to the troop at checkout. A link to the physical form is ilisted below. Physical forms are also available online or at the Indian Nations Council located at 4295 S. Garnett Road, Tulsa Oklahoma 74146. We strongly recommend that all physical
forms be collected by the troop’s adult leader two weeks prior to camp
to ensure that all scouts have a properly completed form. There is no charge for medical services
performed in camp for scouts and leaders, except for certain medical
supplies. All campers needing
services performed at the hospital/clinic are required to handle payments
through the troop of family insurance policy. Although the Health Lodge is open 24 hours
a day, please limit non-emergency visits to those times in the morning and
evening dedicated for sick call. Annual Health and Medical Record
